Understanding GLP-1 agonists: the science behind medical weight loss & metabolic health

What are GLP-1 agonists?

GLP-1 (glucagon-like peptide-1) is a naturally occurring hormone produced in the gut after you eat. Its primary job is to tell your brain, “I am full,” and to signal your body to release insulin to handle the sugar from your food. In many patients struggling with weight, this natural signal is blunted or ineffective.

Modern medical treatments use GLP-1 receptor agonists to mimic this natural hormone, but with a longer duration of action. You have likely heard them referred to by their active ingredients or brand names: semaglutide, found in Ozempic® (approved for type 2 diabetes) and Wegovy® (approved for weight management), and tirzepatide, the active ingredient in Mounjaro®, which acts on both GLP-1 and GIP receptors for potentially more potent metabolic effects.

How do these treatments work?

These medications do not “melt fat” directly. Instead, they correct the underlying metabolic dysfunction that causes weight gain, through three primary mechanisms.

Slowing gastric emptying: by slowing down how fast food leaves the stomach, you feel physically fuller for longer after smaller meals. Regulating insulin: they improve insulin sensitivity and blood sugar control, shifting the body from “fat storage” mode to a more metabolically flexible state. Targeting the brain: GLP-1 receptors are found in the brain’s appetite centres, directly influencing how you think about food.

Beyond the scale: mental health and the alcohol connection

One of the most profound benefits of GLP-1 therapy is psychological. Many patients suffer from constant, intrusive thoughts about food — what to eat next, guilt over what was just eaten, or fighting intense cravings. This “food noise” is a significant mental burden that depletes willpower and increases anxiety. GLP-1 agonists work on the brain’s reward centre to turn this volume down, and patients often report a newfound sense of mental freedom.

Interestingly, the same dopamine reward pathways that drive sugar cravings also drive the desire for alcohol. Many patients on GLP-1 therapy report a spontaneous reduction in their desire to drink — a dual benefit that removes empty calories and reduces the inflammatory stress alcohol places on the liver and metabolism.
